How the DSHS Referral
Process Works

1. Send the referral information to the DSHS Transition Coordinator (contact above).

2. DSHS reviews the individual’s needs to determine the appropriate placement.

3. If Pacific Ridge is identified as a fit, DSHS will notify us and coordinate next steps.

4. Pacific Ridge Intake Review:

  • We confirm eligibility (must be 18+)

  •  We review clinical history and service needs

  •  We contact the referring party/family to begin onboarding.
